The role of rotating shift work in sleep disturbance among healthcare workers at Merwais Regional Hospital

Shift work, particularly rotating shifts, is a type of scheduling system in which workers perform a cycle of different shifts, such as day shift, night shift, weekend shifts, or any swing shifts that the industry may require. In the healthcare industry, employees often work rotating shifts to provide continuous patient care. Study results indicated that working rotating shifts disrupts regular sleep cycles, leaving healthcare professionals exhausted and increasing the risk of several chronic health issues, and can also lower the quality of healthcare services provided to patients. This cross-sectional study examined the sleep quality among different shift workers at Merwais Regional Hospital in Kandahar, Afghanistan. We collected sleep quality data from 181 healthcare professionals who worked three shifts: day, night, and rotating shifts, using the Pittsburgh Sleep Quality Index (PSQI) and the Kruskal-Wallis test. Rotating and night shift workers had significantly poorer sleep quality than day shift workers, as indicated by the statistical analysis of the Kruskal-Wallis test. The findings of this study revealed that rotating and night shifts are strongly associated with increased sleep problems, which can throw off the body’s natural clock and hormone levels, leading to digestive issues, heart problems, reproductive difficulties, and a higher risk of certain cancers. This research aims to evaluate the sleep disturbances associated with shift work and their medical impact in the challenging and resource-limited context of Afghanistan’s healthcare system.

:چکیده
نوبت‌کاری، به‌ویژه نوبت‌کاری چرخشی، نوعی سیستم زمان‌بندی است که در آن کارمندان در دوره‌های متفاوتی از نوبت‌ها مانند نوبت روز، شب، آخر هفته یا نوبت‌های ترکیبی مطابق با نیاز سازمان فعالیت می‌کنند. در بخش خدمات صحی، کارمندان اغلب برای تأمین مراقبت پیوسته از بیماران، در نوبت‌های چرخشی کار می‌نمایند. مطالعات پیشین نشان داده است که نوبت‌کاری چرخشی چرخهٔ منظم خواب را مختل می‌کند، سبب خستگی کارمندان صحی می‌شود، خطر ابتلا به بیماری‌های مزمن را افزایش می‌دهد و کیفیت خدمات صحی ارائه‌شده به بیماران را کاهش می‌دهد.
این مطالعهٔ مقطعی کیفیت خواب کارمندان صحی را که در نوبت‌های مختلف در شفاخانهٔ حوزه‌ای میرویس در قندهار کار می‌کنند، ارزیابی کرده است. داده‌های کیفیت خواب از ۱۸۱ کارمند صحی که در نوبت‌های روز، شب و چرخشی فعالیت داشتند، با استفاده از شاخص کیفیت خواب پیتسبورگ (PSQI) گردآوری شد. برای مقایسهٔ کیفیت خواب میان گروه‌ها از آزمون کروسکال–والیس استفاده گردید. نتایج تحلیل آماری نشان داد که کارمندان نوبت شب و نوبت چرخشی کیفیت خواب به‌مراتب پایین‌تری نسبت به کارمندان نوبت روز دارند.
یافته‌ها نشان می‌دهد که نوبت‌کاری شب و چرخشی به‌طور معنادار با افزایش اختلالات خواب مرتبط است و می‌تواند ریتم طبیعی بدن و تعادل هورمونی را برهم زند، که این امر منجر به مشکلات گوارشی، قلبی، تولید مثلی و افزایش خطر برخی از انواع سرطان می‌شود. این پژوهش بر اهمیت پرداختن به اختلالات خواب ناشی از نوبت‌کاری و پیامدهای صحی آن در چارچوب نظام صحی محدود منابع افغانستان تأکید می‌نماید.
